About the role

The Lung Cancer Program Coordinator (LCPC) is responsible for the full continuum of care from lung cancer screening to detection, diagnosis, treatment and survivorship. Additionally, the LCPC oversees the development, implementation, and maintenance of all aspects of the Lung Health Programs including Low Dose CT Screening, patient navigation, incidental lung nodule findings and management. The overall goal is to ensure cancer patients are effectively and efficiently navigated through the health care system receiving appropriate care throughout the continuum to optimize support, educational services and to provide the communication link for coordination of care among the multidisciplinary health care team. Responsible for establishing, building and maintaining strategic relationships with our physician practices, which includes promoting, educating and marketing the Advent Health Lung Health Program to our employed physicians, community physicians and their office staff. The value you'll bring to the team: Understands the navigation business model and builds relationships with community physician practices/groups to grow disease-specific inpatient and outpatient market share. Researches and develops tools and strategies to promote the multidisciplinary lung health program to area physicians. Visits physician offices regularly, handles physician/physician office issues promptly, and initiates physician communications as needed. Assures physician satisfaction with services and presents concerns to administrative leadership on an ongoing basis. Collaborates with physicians and administrative leadership to establish, implement, and evaluate goals related to growth, volume, clinical outcomes, and revenue.

Responsibilities

Requirements

  • Associate's of Nursing
  • 3 years of Work Experience
  • Registered Nurse (RN)
  • Basic Life Support - CPR Cert (BLS)
